Thank you for choosing our office for all your dental needs.

We have created a comfortable environment where you can feel confident in the level of dental care you receive. When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office.

Your initial exam will last approximately one and a half hour. This visit will help us get acquainted with each other and learn about your goals and desires for your dental treatment. Generally, X-rays will be taken allowing your doctor to examine the jaw’s structure, teeth position and teeth decay.

Oral hygiene instructions will also be provided along with suggestions to help you care for your teeth.

We believe that good dental care begins with open communication. We promise to speak candidly with you about any symptoms present that may require further dental treatment, our diagnosis, alternative treatments that may exist, and our recommendations. Working together, we can achieve a true partnership, with one common goal–keeping your smile beautiful and your teeth, gums, and jaw joints healthy!

  • 20% savings on all non-elective cosmetic dentistry procedures, including treatments for periodontal disease.
  • 20% savings on all elective cosmetic dentistry procedures, including Invisalign and porcelain veneers (minimum of four teeth).
  • No maximums, waiting periods, exclusions or red tape with the Patient Choice Plan.
  • 75% savings on professional teeth whitening plus FREE whitening forever services, which includes one free whitening gel syringe at every attended hygiene visit. (This offer does NOT include KOR whitening system.)
  • 10% Savings on all dental products.

Annual fees for membership in the

  • 4 consecutive monthly payments of $108 (total $432)
    Ages 14 & up
  • 4 monthly payments of $64.25 (total $257)
    Children 13 & under

Note: Additional family members or co-workers may be added for $297 each, with a minimum of three or more additional members.

“Patient” agrees to:

  • Attend all scheduled appointments at the scheduled times.
  • Give minimum two business days’ notice for rescheduling appointments.
  • Comply reasonably with “Dentist’s” clinical recommendations.
  • Make on-time payments (maximum savings achieved by paying in full).
  • Provide occasional feedback to help us improve our services.
  • Refer friends, family, coworkers, neighbors, etc. if happy with our services.
